Treatment options for massive rotator cuff tears.

چکیده
Various classifications of rotator cuff tears have been proposed. Many authors currently define a tear as massive if there is a detachment of at least 2 complete tendons. Whereas small tears can occur without symptoms, massive tears are always associated with weakness and, especially in young patients, often with painful disability. Massive tears are only rarely due to an acute injury; rather, they are usually chronic and are associated with myotendinous retraction, loss of musculotendinous elasticity, fatty infiltration of muscles, static (superior) subluxation of the humeral head, and ultimately, osteoarthritis. If fatty infiltration of the respective muscles is beyond Goutallier stage 2 and/or if there is cranial migration of the humerus resulting in an acromiohumeral distance of less than 7 mm, the probability that successful cuff repair can be achieved becomes so low that these massive tears are called irreparable. Whereas small tears with no or little retraction relatively frequently remain small, large, reparable tears usually increase in size and can rapidly become irreparable with no further increase in pain or disability. It is therefore important to determine the definitive treatment of a massive, reparable tear at the time of its identification. The key parameters that are decisive for the definitive treatment are the patient’s symptoms, reparability of the lesion, and shortand longer-term functional demands.

Background: Recurrence rate of rotator cuff tears is still high despite the improvements of surgical techniques, materials used and a better knowledge of the healing process of the rotator cuff tendons. Large to massive rotator cuff tears are particularly associated with a high failure rate, especially in elderly. Augmentation of rotator cuff repairs with extracellular matrix or synthetic patc...

Background: The alternative surgery for massive and irreparable shoulder rotator cuff tears in older patients is thedebridement of subacromial bursa, biceps tenotomy, and tuberoplasty (reverse acromioplasty). This study aimed toreport the effectiveness of such a treatment performed arthroscopically in a small group of patients for a short periodof time. Methods: This pros...

Massive irreparable rotator cuff tears in young patients are a particular challenge for the orthopaedic surgeon. Surgical treatment options include debridement, partial rotator cuff repair, patch-augmented rotator cuff repair, bridging rotator cuff reconstruction with graft interposition, tendon transfer, and reverse total shoulder arthroplasty. Recently, reconstruction of the superior glenohum...
متن کاملOperative Technique: Arthroscopic Repair of Massive Rotator Cuff Tears
Introduction Rotator cuff tears are a common cause of pain and disability. 2 With modern advances in arthroscopy and arthroscopic techniques, most rotator cuff tears can now be repaired arthroscopically, with several authors reporting successful results. However, the optimal management of massive rotator cuff tears is controversial and evolving. In general, a massive rotator cuff tear (MRCT) is...
